I acknowledge the contagious nature of COVID-19 and that my child(ren) and I may be exposed to or infected by COVID-19 by participating in VBS activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Ben Hill UMC may result from the actions, omissions, or negligence of myself and others, including, but not limited to, parish employees, volunteers, and program participants and their families. By signing this agreement, I also understand the procedures and protocols used during Vacation Bible School. -I will take my child's temperature BEFORE coming to VBS. -I will not allow my child to attend VBS if feeling sick or having been exposed to a positive case of COVID-19 -I will send a mask with my child and encourage my child to wear a mask at all times -When signing in my child each day I am stating that my child is without any symptoms of COVID-19 -I will follow all other procedures set by the VBS team. -I understand that procedures may change
